MNHC is seeking a driven, detail oriented, bilingual (English/Spanish) COVID-19 Community Health Worker to join a patient-focused, primary care-centered COVID-19 Testing and Vaccination team at a large inter-disciplinary community health center in San Francisco’s Mission District. This position ultimately represents the patient and community, and provides COVID-19 education and information to patients and community members at our outdoorCOVID-19 Testing and Vaccination Site, ensure COVID-positive patients and families successfully access and navigate needed care and community resources (i.e. food access, rental assistance, legal aid, etc), conduct outreach to the community about COVID testing options and testing criteria.


Essential Functions and Responsibilities:

  • Screen COVID+ patients for necessary community resources and/or support.
  • Follow up with and navigate COVID-positive patients in accessing necessary resources (i.e. food access, rental assistance, legal aid, etc). Provide support to patients and families.
  • Build and maintain trusting relationships with local organizations. Attend community meetings, as needed.
  • Coordinate with community partners conducting COVID-related outreach.
  • Maintain and distribute list of community resources available to patients affected by COVID-19.
  • Represent MNHC at community events, share and distribute reliable information to combat rumors and fears of COVID, addressing socioeconomic and language barriers.
  • Screen patients, guests, staff for COVID symptoms, as needed.
  • Field questions from patients about COVID testing and vaccinations.
  • Use EHR to document patient registration, encounters, results, patient tracking.
  • Participate in COVID result notification workflow under direction of Lead RN.
  • Screen COVID+ patients for necessary community resources and/or support (i.e. rental assistance, food access). Stay informed of community resources, coordinate with partners and refer patients as needed.
  • Assists in contact tracing by calling patients with a positive COVID test. Provide support and gather information. Interview and provide support to close contacts within the households.
  • In-reach to patients who haven't been vaccinated.
  • Assist with paperwork and necessary documentation for patients being tested and vaccinated.
  • Set-up and take down of testing and vaccination sites and organization of associated supplies.
  • Other additional duties as assigned.


Qualifications & Requirements:

  • Bilingual English/Spanish required. Fluency in other languages preferred.
  • High school diploma or equivalent required.
  • 1-2 years of health care, case management, or patient advocacy experience strongly preferred.
  • 1 year of customer service experience required.
  • Flexibility to work at different MNHC locations and/or community locations as needed, and to work outdoors in testing tents.
  • Reliable transportation and/or access to vehicle.
  • Ability to interact with persons of all cultural, education, and socioeconomic levels.
  • Intermediate computer experience required (we use Office 365, EHR)
  • Flexible schedule which may include evenings and weekends.
